New (or Lost) Passport Instruction Sheet

For individuals who:

  • have never had a passport; or
  • lost a valid passport; or
  • whose passport was issued more than 15 years ago; or
  • whose most recent passport is damaged or mutilated; or
  • whose most recent US passport was issued prior to 16th birthday

What You Need:

Passport Requirements: Certified birth certificate or naturalization certificate or passport issued over 15 years ago.
Passport Photo Requirements: Two identical passport photos (2x2 with white background). Click here for details.
Passport Application Forms: Completed DS-11 Form.This form must be completed online and printed on two pages (not back to back). The passport office will not accept hand-written DS-11 applications. Print 2 copies of your completed DS-11 form; one form must be endorsed and enclosed inside a sealed envelope and one copy attached to the outside of the sealed envelope. If the applicant has lost a valid passport, please also complete DS-64 Form. Please note; once the DS-11 form is submitted on-line, the lost passport will be automatically canceled by the US Department of State.
Letter of Authorization: Two signed Letters of Authorization. Please select the authorization letter that matches the processing location and print two copies. Both must have original signatures, one will be enclosed inside the sealed envelope and one attached to the outside of the sealed envelope. (For minor children, both parents must sign these forms).
Proof of Departure: Computer generated flight itinerary from the airline or a travel agency, copy of airline ticket or a corporate letter stating US departure by private aircraft. One copy should be attached to the outside of the sealed envelope.
Additional Requirements: Please Note: If the applicant is submitting a damaged or mutilated passport for replacement, a signed statement addressed to the US Department of State must be included explaining the condition of the passport.
Global Reservation Form and Fees: Complete and print the Global Reservation Form (Order Summary Page) and include all necessary fees. Please attach the Global Reservation Form to the outside of the sealed envelope.

US Government Fees: PLEASE NOTE: New US Government Passport Fees go into effect July 13th, 2010
  • Fee Effective July 13th, 2010:
    Adults - $170.00 payment for the passport in the form of a check made out to the U.S Department of State and included with the DS-11 form inside the sealed envelope, and a separate $25.00 payment to the Post Office or the Clerk of Court.
    Minors - $140.00 payment for the passport in the form of a check made out to the U.S Department of State and included with the DS-11 form inside the sealed envelope, and a separate $25.00 payment to the Post Office or the Clerk of Court.
  • Processing Time: Passports can be processed as Emergency Same Day or Next Day; Rush 48 Hours or Rush 3-10 Business Days.
    For Emergency Same Day/Next Day processing, you must call to Travisa to make a reservation for your passport submission date.
    Please Note: the US Passport Agency does not currently offer rush processing of passport cards. For those customers who request both a passport and a passport card, the passport card will be mailed separately to the customer at a later date. In some cases the US Passport Agency mails the cards directly to the applicant. In cases where they mail the card to Travisa, additional shipping fees will apply.

    Mailing Instructions:

    1. Keep one photcopy of each item listed above, and take the original documents to a Clerk of Court or Authorized Post Office and ask them to endorse the documents and seal them in an envelope marked "TO BE OPENED BY PASSPORT OFFICE PERSONNEL ONLY." This sealed envelope must be presented to the passport office within 5 working days from execution.
    2. Do not write checks until directed to do so by clerk. THE GOVERNMENT FEE (Check made out to "US Department of State") MUST BE INSIDE THE SEALED ENVELOPE.
    3. You must present proof of identity such as a driver's license (which has been issued for at least 6 months). A copy of both sides of the photo ID must be included inside the sealed envelope.
    4. MINORS - All minors must appear in person at the post office or the courthouse. Minors (under the age of 16) must be accompanied by BOTH PARENTS or legal guardian with acceptable proof of ID. A photocopy of both parents’ identification must be provided. If only one parent appears the other parent must sign a Statement of Consent Form DS-3053.
    5. Do not leave documents with clerk.
    6. Attach one copy of the DS-11 form, one signed letter of authorization, a copy of the travel itinerary, a copy of the photo ID and the Global Reservation Form order summary to the outside of the sealed envelope.
    7. Send the Global Reservation Form order summary plus sealed envelope to TRAVISA for processing.
    8. You must call Travisa in advance to reserve a space for processing if you need emergency service.
