Passport Requirements: Valid/signed U.S. passport.
Passport Photo Requirements: No photos required.
Passport Application Forms: One completed application form DS4085This form must be completed online. The passport office will not accept hand-written DS-4085 applications.
Proof of Departure: Computer generated flight itinerary from the airline or travel agency, copy of airline ticket or a corporate letter stating U.S. departure by private aircraft.
Additional Requirements:
Global Reservation Form and Fees: Complete and print the Global Reservation Form (Order Summary Page) and include all necessary fees.
US Government Fees: PLEASE NOTE: New US Government Passport Fees go into effect July 13th, 2010
Fee Effective July 13th, 2010: $142.00 payment included on the Global Reservation form or payable in the form of a check made out to the U.S. Department of State.
Processing Time: Passports can be processed as emergency, Same Day/Next Day (depending on U.S. departure date) or rush, 3-10 business days. For emergency Same Day/Next Day processing, you must call to Travisa to make a reservation for your passport submission date.
