Requisitos Del Pasaporte: Valid/signed U.S. passport.

Requisitos De la Foto Del Pasaporte: Not required.

Formularios De Inscripción Del Pasaporte: DSP-19 Passport Form, completed and signed.

Letra de la autorización: Signed Letter of Authorization.

Prueba de la salida: Copy of computer generated flight itinerary from the airline ticket or travel agency, copy of airline ticket or a corporate letter stating US departure date by private aircraft. You must call to reserve a space for processing if you need same day service.

Requisitos Adicionales: Court-Certified Name Change Decree OR Marriage Certificate (Not a Marriage License) OR Divorce Decree, in the form of a certified copy.

Formulario Orden de Servicio lleno y Honorarios y pagos: Complete and print out the service order form and include all necessary fees. Must include a check for $60.00 payable to US Passport Services.

Tiempo De transformación: Passports are processed according to U.S. departure date.
