Renewal of Second Valid Passport Instruction Sheet

What You Need:

Passport Requirements: 1) Valid/signed U.S. passport. 2) Valid/signed 10 year U.S. passport.
Passport Photo Requirements: Two passport photos (2x2 with white background). Click here for details.
Passport Application Forms: One completed application form DSP-82. This form must be completed online. The passport office will not accept hand-written DS-82 applications.
Proof of Departure: Computer generated flight itinerary from the airline or travel agency, copy of airline ticket or a corporate letter stating US departure by private aircraft.
Letter of Authorization: Two signed Letters of Authorization.
Additional Requirements: A letter, signed by the applicant, addressed to the Passport Office, explaining why a second passport is necessary.
Global Reservation Form and Fees: Complete and print the Global Reservation Form (Order Summary Page) and include all necessary fees.

US Government Fees: PLEASE NOTE: New US Government Passport Fees go into effect July 13th, 2010
Fee Effective July 13th, 2010:
  • If you are submitting Form DS5504, $60.00 payment included on the Global Reservation form or payable in the form of a check made out to the U.S. Department of State.
  • If you submitting Form DS82, $170 payment included on the Global Reservation form or payable in the form of a check made out to the U.S. Department of State.

    Processing Time: Passports can be processed in the San Francisco jurisdiction in approximately two to three weeks.

    Mailing Instructions:

    1. Complete and print out all forms.
    2. Check all information for accuracy.
    3. Mail the Global Reservation Form with all of the items above to Travisa. Processing is available in the following location(s):
